Before your visit
- Bring a referral, if your insurance plan requires one
- Bring prior imaging (X-ray / MRI) on disc or via your patient portal
- List current medications, allergies, and prior surgeries
- Bring your insurance card and a photo ID
- Arrive 15 minutes early to complete new-patient paperwork, or fill it out ahead of time using the forms below

What to expect
After check-in, you may be scheduled for pre-visit X-rays. If you've already had X-rays elsewhere, please bear with us — for more complex injuries, prior X-rays sometimes don't give Dr. Nascimento enough information, and we want to make sure we have everything needed to give you the best opinion possible.
Once Dr. Nascimento is ready to see you, our medical assistant staff will bring you to the exam room, update your chart, and take a brief history.
Dr. Nascimento will then take a more in-depth history, perform a thorough physical exam, and review all relevant imaging with you. Using a shared decision-making approach, he'll help you choose the best treatment plan for your situation.
Depending on your case, this may also mean more advanced imaging and a second visit, either virtual or in person.
Helpful things to bring
- All imaging studies on CD — a radiology report is helpful, but doesn't always tell the whole story; the actual images are always more useful than the report alone
- For second opinions: prior clinical notes, especially operative notes, sent to the office ahead of time or brought with you to your visit

Dr. Nascimento has extensive experience treating patients injured on the job. The goal is always to treat conservatively but efficiently, to get you back to work safely with as little time lost as possible.

All shoulder and knee injuries, including but not limited to:
- Shoulder labrum tear / dislocation
- Rotator cuff injuries
- Biceps tendon injuries
- Impingement
- Frozen shoulder or limited range of motion
- Meniscus tear
- Knee instability / ligament tears
- Cartilage injuries
- Knee swelling, catching, or locking
After your initial consult, Dr. Nascimento's staff will communicate with your workers' comp adjuster promptly and keep you informed at every step of the process.
